Job Description:
A company is looking for a Case Manager, Social Worker to
evaluate beneficiaries' needs and coordinate healthcare
services.
Key Responsibilities
Assess and develop individualized care plans for beneficiaries
Coordinate care among beneficiaries, providers, and healthcare team
members
Manage caseload and maintain documentation in compliance with
regulations
Required Qualifications
Bachelor's degree in Social Work
Active, unrestricted Social Work license
Minimum 3 years of experience with older adult populations in
healthcare
3-5 years of clinical experience, including 1-3 years in case
management review
Proficiency in Microsoft Office applications
